Candidates should have passed +2 examination of the HSC/CBSE/ISC or equivalent examination with be following subject combination's (a) Physics, Chemistry and Biology or (b) Physics, Chemistry, Botany and Zoology (c) Physics, Chemistry, Biology and Mathematics (With minimum 70% aggregate in the above subjects for MBBS)

The form can be obtained in person/through post/may be downloaded from the University website. If the form is downloaded, the completed form must be accompanied by a D.D. as per the above instruction.
